Consultants and hospital doctors — NHS pay, private practice and the company if you need one

A consultant file is often a trust salary, waiting-list extra sessions, private practice and a pension statement that triggers an annual allowance charge. We put those streams on the right returns and tell you if a limited company still helps once IR35 and the NHS job are in the picture.

Private practice beside an NHS job

Fee books, practising privileges and room costs belong on the private side. The NHS job stays on PAYE. We do not bury private income in a trust P60, and we do not run private costs through the NHS expenses code. If you incorporate the private work, formation is £150 and the Locum & Medical package (or a quoted practice package) takes the company.

  • VAT check — most clinical fees are exempt; cosmetic or mixed supplies may not be

Annual allowance and the tax charge

NHS pension growth can create an annual allowance charge even when you feel cash-poor. We work from the statements the scheme sends. Scheme Pays, carry forward and the tax return entries are accounting work. Whether you should change contribution rates or take other pension steps is a decision for you and, where needed, an independent financial adviser. We will not sell a product.

Waiting-list initiatives and extra sessions

Some extra sessions are PAYE. Some are invoiced. We treat them as they actually paid you, then put the total on the right return so the pensionable/non-pensionable split is not guessed in March.

Choose the structure that fits the work

Keep private work in your own name if

Fees are modest, you want one Self Assessment, and a company would cost more in filings than it saves. We still keep a proper fee book.

Use a company if

Private turnover, staff or limited liability make a company the cleaner vehicle, and IR35 on any public work has been looked at. Formation is £150.
